Maybe you misunderstood the example.
program ("/usr/local/sbin/syslog-ng-mailer.sh $HOST"
is bad, the whole string including the space and $HOST is treated as the program name... Instead of that use
program ("/usr/local/sbin/syslog-ng-mailer.sh" template(...
